2025 Gala: Submit Your Table Guests' Info Here

Please read these instructions carefully:

• Please fill out this form for yourself and all guests you are bringing with you the night of the gala.
• Please only fill in guest information for the number of tickets you purchased.
  • For example, if you purchased 4 tickets, you need only complete information for yourself and 3 guests.
  • If you purchased more than one table, please complete the form anew for each additional table. Reload this page to refresh the form.
• If you do not know the name of each respective guest that you would like seated at your table, they can be added later. However, all data must be provided no later than Wednesday, September 24.
• We will update this information in our database to make sure everyone in your party has an assigned seat.
• If we do not have specific seats requested for your respective guests, OCI reserves the right to assign those seats to gala volunteers or other guests as OCI deems necessary.
